As a board-certified Nurse Practitioner and dedicated athlete, I am committed to delivering evidence-based, patient-centered care at Lake Grove Family Medicine. I have a particular interest in weight management and preventive health, with a focus on supporting patients in achieving sustainable lifestyle changes that improve overall well-being. By integrating my clinical expertise with an appreciation for the role of physical activity and nutrition, I strive to help patients optimize their health and reach their fullest potential.

​Thank you for your interest in our Weight Loss Program. Please download, review, and sign our Agreement and Consent for Weight Management and Weight Loss Financial Agreement. Once this is done, please email these documents to admin@lakegrovefamilymedical.com along with the following information.

If you are new to our office, please also provide the following:

  • Your DOB –
  • Your address –
  • Your Phone number –
  • A copy of your insurance card (if we will be billing your insurance).

Once this is received, our office staff will reach out to get you scheduled.
